This form may be used by Cedar Creek homeowners to report a concern of the applicable Covenants, Conditions & Restrictions.

Submitting a report does not constitute a determination that a violation has occurred. Reports will be reviewed and, when appropriate, the property will be independently inspected before enforcement action is taken.

Please provide factual information about the condition you observed rather than assumptions regarding the homeowner or the reason the condition exists.

This form should not be used to report emergencies, criminal activity, or matters under the jurisdiction of the City of Olathe or other governmental agencies.
